Our Survey
Our survey is an extremely important part of our Transparency Project.
The survey is designed to evaluate the number of interventions during your labor and your satisfaction with your birth experience.
Any woman who lives in Adair, Scotland, Schuyler, Putnam, Macon, Randolph, Sullivan, Linn, Shelby, or Knox county and who gave birth in 2003, 2004, 2005, 2006, 2007, or 2008 is eligible
to take the survey. It should take less than 20 minutes to complete. Your answers will be combined with other responses and will help us
in identifying areas of local maternity care that need improvement.
The survey is available online, or you can download a copy, print it out, and mail it to us. We will also have copies
available weekly at the Kirksville Farmer's Market. Please only take the survey once per birth.
If you would like to share more information about your birth experience or would like to add additional information not requested
in the survey, please contact us.
